Athletic Participation Forms
Any athlete signing up for a sport must complete all of the appropriate forms, have a parent or guardian sign them and submit them to the athletic office on or before the announced date before the season begins. The DEADLINE for Spring SPORTS is Tuesday, February 28, 2012. THE SPRING SEASON BEGINS ON MARCH 2, 2012.
All forms are available online and at the athletic office.
