PLEASE COMPLETE THE SCREENING CHECKLIST EVERY AM FOR YOUR STUDENT ATTENDING

IN-PERSON LEARNING.

CAN I ATTEND SCHOOL?

IF you can answer NO to ALL symptom questions and NO to ALL contact exposure questions then your student can report for in-person learning.

Upon arrival at school, additional temperature screening will be conducted.  Any students with a temperature greater than 100.4 will need to have a parent/guardian return to pick up student and follow school policy regarding illness management. 

Hazlet District created a COVID-19 tab on our main website for any information the district receives from NJDOE, Monmouth County DOH, NJ DOH, CDC etc. Please click COVID-19 to be re-directed there.

RAINE INFO 2020 ASSISTANCE --During the Covid-19 Pandemic, RAINE will not be functioning as we have in the past. Deliveries may not be possible and resources may be limited so please keep that in mind as you fill out the form.      The Hazlet Twp School Nurses have teamed up with the Raine foundation to reach out to families that may be in need of assistance during the coming Holidays.  If your family would like to request help from the Raine Foundation for either Thanksgiving or Christmas, please click on the link below and complete the document ASAP.  The Raine Foundation will not be using paper copies or mailings this year.  If you need assistance with this form, please contact your school nurse.
