Diagnosis and treatment of illness, including the prescription of drugs, is not the responsibility of the school.
Students requiring medication at school, prescription and over-the-counter, must have a written order from their physician stating the name of the medication, purpose, dosage, time to be administered, and side effects.
Written permission from the parent is also necessary. These forms are available from the school nurse.
Parents shall directly deliver medication to the school nurse in the original labeled prescription container from the pharmacy.
They are to pick up any remaining medication at the end of the school year.
If your child has a life-threatening illness such as asthma, diabetes, severe bee sting allergy, etc., please contact your school nurse regarding their treatment plan.
