  • Maschio’s Allergy Management Web Information ---Please email me lnicholl@hazlet.org to obtain a full copy of the Food Allergy Management Program Parent Packet for the 2024-2025 school year. Per the USDA, a non-life threatening allergy may be considered a disability, requiring a meal modification if it impacts a major bodily function or other major life activity.

    The enclosed medical statement (or similar medical documentation) is required to be completed annually by a medical professional. Documentation may also be provided for students with Celiac disease, diabetes, lactose intolerance, chewing/swallowing disorders, or any other special dietary need not listed above. 

    Families may submit their medical statement (pages 2-3 of the parent packet) by:

    • Completing it online: www.bit.ly/safemenu

    • Emailing the forms to nutrition@maschiofood.com with the subject line “SAFE MENU”

    • Faxing forms to the nutrition department at (908) 888-2335

    *Note for POS Systems (i.e Genesis, etc.) - Allergy alerts on the POS should not be the sole safety measure for students with life-threatening food allergies as there may be hidden allergen ingredients in some menu items. We encourage students with life-threatening food allergies to be provided with a safe menu.
