Athletics' Online Registration
GUIDELINES:
If you played a fall/winter/spring sport last year after clicking the link below, you are to Log-in (not sign-up) then hit the Winter Registration tab for either Hazlet Middle School or Raritan High School. The majority of information will re-populate and parents will only have to sign-off on policies electronically. Head coaches will receive a daily email with a list of who has successfully registered.
New athletes who did not play a fall/winter/spring sport (or participate last year) will need to Sign-up and follow the directions to the Winter Registration Tab. If you have a family account you should log-in and add the new student athlete.
All athletes will need current Physical Examination Forms on file in the Nurse’s Office. Blank forms can be found online, on the Family ID web page and are also available in the HMS or RHS Nurse’s Office as well as the Athletic Office.
As per the NJSIAA all Physicals are good for one year from the date administered. If you have any questions please send the student to the nurse. No paperwork is to be handed in to the Athletic Office.
Any athlete signing up for a sport must complete all of the appropriate forms on our online registration link above when the registration window is open. Only the annual physical form will be handed into the SCHOOL NURSE.
Please be aware that athletes returning from COVID-19 will need a clearance note from a physician when they return to sports.
